We are part of a countrywide network of community, migrant, homeless, and public housing health centers that serves over 18 million people. PCHC, in particular, we are an integral part of the Providence community and the largest of eight health center organizations in Rhode Island that provides care to over 122,337 patients in Rhode Island with PCHC caring for approximately one third.
PCHC employs Highly Qualified Medical Staff
We employ highly skilled, compassionate, and culturally diverse workforce of pediatricians, internists, nurse practitioners, nurses, and certified nurse midwives. Our Physicians are board-certified. A physician who is board-certified has completed four to seven years of full-time accredited training beyond medical school, and has successfully passed exacting written and oral examinations in his or her specialty.

What is a Patient-Centered Medical Home?

A patient-centered medical home is health care focused on you, the patient. It is a partnership between you and your medical home team, including your primary care doctor. As you require additional services from specialists and other healthcare providers, they’ll become part of your medical team as well. Therefore, PCMH is made up of health care professionals committed to improving your overall health and helping you reach your health goals.

We have fully implemented the Electronic Health Records (EHR) which help our providers and patients work together and, most importantly, help our providers deliver the best possible care.
