The Providence Community Health Centers at North Main offers patients coordinated physical, mental health, and substance use care under one roof. A personal physician provides patients with continuous comprehensive care for adults from regular check ups to treatment for chronic illnesses like diabetes and high blood pressure.  If you wish to use Providence Community Health Centers at North Main Street, you must choose Dr. Memon as your Primary Care Physician.
