Thank you for your interest in employment opportunities at Providence Community Health Centers.
To be considered for employment, you must be able to complete the entire application for employment, providing ALL the information requested including the following:
- Your current telephone number.
- All past employment.
Employment experience information must include:
-From and To dates (month and year)
-Reason for leaving
-Supervisors full name
-Employer’s telephone number - Your bilingual skills.
If you are bilingual, identify the language spoken other than English and the level by circling little, fairly or well. Make sure that you complete a language test if your other language is Spanish, Portuguese, Cambodian or Laotian.)
If you do not have the information required to submit a completed application, do not complete an application at this time. Please return when you can provide ALL of the required information. Incomplete applications will not be considered.
All complete applications will be reviewed and retained for 90-days. If we have an opening during that period of time and your application indicates that you may have the required skills and experience, we will call and ask that you come in for an interview.
