Who we are and what we do
Providence Community Health Centers (PCHC) is a non-profit Federally Qualified Health Center that provides quality primary health care services to more than 80,000 residents of Rhode Island’s capital city.
Established in 1968, we are one of the largest primary health care providers in Rhode Island and treat patients regardless of their ability to pay.
Our services include Family Practice, Adult Medicine, Pediatrics, Obstetrics, Gynecology, Behavioral Health, Optometry, Dental Care, Express Care, and a number of specialty services.
PCHC is the largest of nine health center organizations in Rhode Island that provides care to almost 200,000 patients in the state. We are also part of a national network of community health centers that serves over 30 million people.
A local board of directors, more than half of whom are patients, governs PCHC.

PCHC is part of a Statewide and National Community Health Center
We are part of a countrywide network of community, migrant, homeless, and public housing health centers that serves over 18 million people. PCHC, in particular, is an integral part of the Providence community and the largest of eight health center organizations in Rhode Island that provides care to over 122,337 patients in Rhode Island with PCHC caring for approximately one third.
PCHC employs Highly Qualified Medical Staff
We employ highly skilled, compassionate, and culturally diverse workforce of pediatricians, internists, nurse practitioners, nurses, and certified nurse midwives. Our Physicians are board-certified. A physician who is board-certified has completed four to seven years of full-time accredited training beyond medical school, and has successfully passed exacting written and oral examinations in his or her specialty.
PCHC is Committed to Serve All Members of Our Community
We continue to be the safety net provider for residents who face major financial, social, cultural, and language barriers, individuals and families experiencing homelessness, and those living in public housing.

- We are certified as a Level 3 (the highest) Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).
What is a Patient-Centered Medical Home?
“You and your health care are at the center of the medical home team.”
A patient-centered medical home is health care focused on you, the patient. It is a partnership between you and your medical home team, including your primary care doctor. As you require additional services from specialists and other healthcare providers, they’ll become part of your medical team as well. Therefore, PCMH is made up of health care professionals committed to improving your overall health and helping you reach your health goals.
- A Patient Portal is fully available to patients in order to assist them to increase their involvement in their own care. We have fully implemented the Electronic Health Records (EHR) which help our providers and patients work together and, most importantly, help our providers deliver the best possible care.
