- Insurance Card (if applicable)
- Identification e.g.,Driver’s License
- Medical Record & Immunization Record
- All medications (e.g., prescriptions, over
the counter medications, herbal remedies, etc.) - Proof of Income: e.g., Pay Stub, W2, letter, etc.
- Birth Certificate
- DCYF Blue Form or Court Documents
