The Providence Community Health Centers at North Main offers patients coordinated physical, mental health, and substance use care under one roof. A personal physician provides patients with continuous comprehensive care for adults from regular check-ups to treatment for chronic illnesses like diabetes and high blood pressure. If you wish to use Providence Community Health Centers at North Main Street, you have the option of choosing Dr. Malik or Dr. Nudelman as your Primary Care Physician.

Starting May 4, patients from our North Main Street health clinic will now receive care at our health center located at 228 Broad Street in Providence. The staff from North Main Street has moved to Broad Street so you will continue to receive care from the same team.
