Who we are and what we do

PCHC services photoProvidence Community Health Centers (PCHC) is a non-profit health care organization and the only Federally Qualified Health Center (FQHC) in Providence, Rhode Island. PCHC provides quality primary health care services that are affordable, comprehensive and culturally sensitive to more than 80,000 residents of Providence and its surrounding areas. Our services include: Family Practice, Internal Medicine, Pediatrics, Obstetrics, Gynecology, Behavioral Health, Optometry, Dental Care and Express Care (urgent care) alongside with selected specialty services.

  • With the well-being of our patients at the top of our agenda, we are dedicated to preventing most common medical problems. Our well-respected medical professionals work to control common chronic diseases, treat acute illnesses, and arrange for referrals to specialists.
  • Established in 1968, we are the largest primary health care provider in Providence and treat patients regardless of their ability to pay. In addition to our eight health centers, we also operate a school-based program, and a dental clinic serving children under the age of 19 and pregnant women.

Through the teamwork of our partners, staff and board members, PCHC ensures that everyone in Providence has access to quality medical and dental care."

— Merrill Thomas, CEO, The Providence Community Health Centers, Inc.

PCHC is part of a Statewide and National Community Health Center

We are part of a countrywide network of community, migrant, homeless, and public housing health centers that serves over 18 million people. PCHC, in particular, is an integral part of the Providence community and the largest of eight health center organizations in Rhode Island that provides care to over 122,337 patients in Rhode Island with PCHC caring for approximately one third.

PCHC employs Highly Qualified Medical Staff

We employ highly skilled, compassionate, and culturally diverse workforce of pediatricians, internists, nurse practitioners, nurses, and certified nurse midwives. Our Physicians are board-certified. A physician who is board-certified has completed four to seven years of full-time accredited training beyond medical school, and has successfully passed exacting written and oral examinations in his or her specialty.

PCHC is Committed to Serve All Members of Our Community

We continue to be the safety net provider for residents who face major financial, social, cultural, and language barriers, individuals and families experiencing homelessness, and those living in public housing.

We Strive for Excellence

  • We are certified as a Level 3 (the highest) Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).

What is a Patient-Centered Medical Home?

"You and your health care are at the center of the medical home team."

A patient-centered medical home is health care focused on you, the patient. It is a partnership between you and your medical home team, including your primary care doctor. As you require additional services from specialists and other healthcare providers, they’ll become part of your medical team as well. Therefore, PCMH is made up of health care professionals committed to improving your overall health and helping you reach your health goals.
  • A Patient Portal is fully available to patients in order to assist them to increase their involvement in their own care. We have fully implemented the Electronic Health Records (EHR) which help our providers and patients work together and, most importantly, help our providers deliver the best possible care.

The Patient Portal has been implemented and is fully available to patients in order to assist them to increase their involvement in their own care.

We will continue our commitment to improve the health outcomes of all the patients we serve!
